My benefits were stolen off my EBT card. What can I do?

If you see an EBT transaction you don’t recognize, call EBT Customer Service at 877-768-5098 right away to cancel your card and get a new one. You should also change your PIN.

You may be able to have your benefits replaced. Here are the steps to request replacement from the Indiana Family and Social Services Administration (FSSA):

  1. Call (317) 234-7989 and leave a voicemail with the following required information:
    1. Full name
    2. Case number
    3. Date benefits were stolen
    4. Amount stolen
    5. Telephone number at which you can be reached
  2. You should receive a call back from the FSSA to confirm the details of your report. This call will come from a 317 area code 10 business days after reporting the theft. If you miss the call, they will leave a voicemail with instructions.
  3. After the phone call, you will be mailed or emailed an affidavit. This is a required form that you sign to attest that the theft occurred as you’ve stated.
  4. Once the FSSA receives the affidavit from you, a decision will be made within 10 business days. If the FSSA finds that you were a victim of electronic theft, you will have benefits replaced.
