My benefits were stolen off my EBT card. What can I do?
- Request a new card and PIN immediately. You can do this through the YourTexasBenefits mobile app, by phone at (800) 777-7328), or at a local benefits office. Note: If your physical EBT card was stolen, unfortunately, the benefits cannot be replaced. In this case, file a police report.
- Fill out Form H1854, Affidavit for Unauthorized Use of Electronic Benefit Transfer Benefits. Important details:
- You have 30 calendar days from the discovery of the theft to submit this form.
- Make sure to fill out all details, including dates, amounts, and retailer/location for each fraudulent transaction.
- Don't forget to sign the form.
- Request a receipt and a copy of the form for your records.
- Your state will review the information to determine your eligibility for replacement benefits within 2 weeks. Please note:
- The review process may take longer, as the form goes through multiple departments.
- You can only receive a maximum of two months' worth of your normal allotment, regardless of the amount stolen.
- You are limited to two replacements per fiscal year.
- To prevent future theft:
- Keep your EBT card frozen when not in use.

Note: TANF funds are treated like cash and cannot be replaced if stolen.
